Intern Confidentiality Agreement


I hereby enter into this Confidentiality Agreement with RAHAB Ministries (RAHAB) for the purpose of preventing the unauthorized disclosure of Confidential Information as defined below. I understand that I may learn or have access to Confidential Information and agree to protect Confidential Information against unauthorized access or disclosure.

1. Definition of Terms.

(a) Confidential Information. For the purpose of the Confidentiality Agreement, Confidential Information shall include but is not limited to:

  1. Identifying information about the individual, including name, contact information, distinguishing physical features (e.g., tattoos, piercings, birth marks, etc.).

  2. Social media content created by or including the individual.

  3. Medical, psychiatric, and psychological reports and records of the individual.

  4. Information regarding the individual’s family.

  5. Any criminal, civil, or juvenile court history.

  6. Any abuse or trauma experienced by the youth.

  7. The street address, county, or location of any of RAHAB’s safe houses; and

  8. Photographs of (i) individuals, (ii) RAHAB’s safe houses, or (iii) staff working at RAHAB’s safe houses.

(b) Individual. For the purpose of the Confidentiality Agreement, the term “individual” shall be used to refer to any person, whether adult or minor, who receives services from RAHAB.

2. Obligations of Intern

Intern shall:

  1. (a)  Adhere to all federal and state laws and regulations regarding Confidential Information.

  2. (b)  Use the Confidential Information only for the purpose of fulfilling the purpose of RAHAB Ministries.

  3. (c)  Hold and maintain the Confidential Information in strictest confidence; Limit e-mailing confidential information when possible

  4. (d)  Agree to store all data on a password-protected computer or in a secure location (e.g., locked file cabinet or drawer, SharePoint)

  5. (e)  Require any employee or any other individual that has or potentially could have access to the Confidential Information to sign a confidentiality agreement and/or release of information

  6. (f)  Maintain confidentiality of all information after termination of employment or volunteer commitment.

  7. (g)  Intern will not disclose or divulge any confidential information in SharePoint.

  8. (h)  Intern will not use any of this confidential information in SharePoint for any purpose other than the performance of job duties.

  9. (i)  Passwords and lock codes will not be disclosed to anyone inside or outside of RAHAB. Changing login credentials is not permitted without written consent from the Director of Minor Mentor Programming.

3. Contact Limitations.

RAHAB Intern agrees to abide by the following rules regarding contact with individuals:

(a) Communication via social media and use of personal cell phone number between individuals and employees/Volunteers not permitted with exception to outreach and community-based programming with the following parameters.

  1. When there is no other form of communication available

  2. Other communication efforts have been exhausted

  3. The program-area Director is aware of the chosen mode of communication iv. RAHAB’s contact information is given in addition to personal contact information

(b) Employee/Volunteer will not give money to individuals directly; other items such as gifts, will be given via RAHAB unless otherwise approved by the program-area Director.

RAHAB Safe Housing Employee/Volunteer agrees to abide by the additional following rules regarding contact with individuals:

(c) Communication, including social media and relationship with individuals after residents are no longer receiving services is not permitted unless given specific written permission by the program Director.

(d) Residents may contact Employees/Volunteers by contacting RAHAB unless given specific written permission by the program Director.

(e) Employees/Volunteers no longer working at RAHAB will not have contact with residents unless given specific written permission by the program Director.

1. Consequences for Breaking Procedure

Interns who improperly use or disclose, or attempt to use or disclose, confidential information will be subject to disciplinary action, up to and including termination of employment and possible legal prosecution, even if they do not actually benefit from the disclosed information.
